2) Date
3) Teacher Name
4) Reason for absence/tardy/checking out early
5) List any and all symptoms your child may have along with their temperature.
Please report any attendance matters within 24 hours of the date.
If you will be checking your child out early or bringing your child late to school, please email us to let us know in advance when possible.
